Care information
The portal supports selected tasks that help patients stay prepared, informed and connected between visits.
Portal features are intended for established, nonurgent care needs and routine administrative activity. Available functions may differ depending on account status, provider instructions and the technology platform used by the practice.
Patients may use the portal for selected communication, documentation and appointment-related needs.
Remove any function below that is not available through the practice’s actual portal system.
Appointment Information
Review scheduled visit detailsView appointment dates, times, visit instructions and telehealth details when those features are enabled. Contact the practice when information appears incorrect or a change is needed.
Nonurgent Messages
Communicate routine patient questionsSend concise, nonurgent questions about existing care, appointments, instructions or other approved topics. Complex concerns may require an appointment rather than a written reply.
Intake and Follow-Up Forms
Complete assigned documentation securelyComplete demographic, consent, screening, symptom-monitoring and other assigned forms before the stated deadline to reduce appointment delays.
Medication Refill Requests
Request review through the approved workflowSubmit the medication name, dose, pharmacy and remaining supply. Refills may require an appointment, monitoring or provider review and are not confirmed merely because a request was submitted.
Selected Records and Documents
Access information made available through the portalDepending on the system, patients may view visit summaries, instructions, letters, laboratory information or other documents released by the practice.
Contact and Account Updates
Keep patient information accurateUpdate permitted contact, pharmacy, insurance or demographic information through the portal and notify the practice when an important change cannot be completed online.
A complete, focused request reduces avoidable follow-up.
Provide the essential details needed to understand and route the request, while avoiding unrelated or excessive information.
Portal availability does not mean every clinical need can be handled through messaging.
New symptoms, significant deterioration, medication changes, diagnostic questions or complex treatment decisions may require a scheduled visit. The practice may respond by recommending an appointment rather than providing detailed care through a written portal exchange.
Portal functions should be used only by the authorized patient or an approved representative with appropriate access. Patients should protect their account credentials and notify the practice when they believe an account or message has been accessed improperly.
Secure portal access depends on verified identity, private credentials and responsible account use.
Patient portal accounts may contain private health, appointment, medication and communication information. Patients should use only their own authorized access, protect login details and follow the approved recovery process when access is lost.
Follow the official activation, login and recovery pathway for the portal account.
Exact steps may differ depending on the portal system and whether the patient is activating a new account, recovering access or requesting authorized representative access.
Account Invitation and Activation
Begin only through the official invitationNew users may receive an activation email, text or printed instruction after the practice verifies their contact information. Follow the official link before it expires and confirm the account details carefully.
Password and Sign-In Protection
Use credentials that are difficult to guessCreate a strong, unique password and use multi-factor authentication when the portal offers it. Avoid saving credentials on shared, public or unsecured devices.
Identity Verification
Confirm the account belongs to the correct patientThe practice or portal may verify the patient’s name, date of birth, contact details or other identifying information before activating, restoring or changing account access.
Forgotten Password or Locked Account
Use the approved recovery processSelect the portal’s password-reset or account-recovery option. Repeated unsuccessful attempts may temporarily lock access and may require identity verification before restoration.
Proxy or Representative Access
Caregiver access should be formally authorizedParents, guardians, caregivers or other representatives may need a separate approved access process. Patients should not simply share their personal password when formal proxy access is available.
Safe Sign-Out and Device Use
Close the session when finishedSign out after each session, especially on a shared device. Keep browsers and devices updated and avoid accessing the portal through unknown links or unsecured public networks.
Small security habits help protect private health information.
Review these steps whenever accessing the portal from a new device, shared environment or unfamiliar network.
Be cautious with unexpected portal emails, text messages and login requests.
Do not enter credentials through an unfamiliar link, download an unexpected attachment or provide verification codes to an unconfirmed caller. Use the verified portal address or contact the practice through its published contact information when a message appears suspicious.
Unable to activate, access or recover the portal account?
Contact the practice through the approved support pathway. Staff may need to verify identity or update account details before access can be restored.
Portal access does not authorize another person to make treatment decisions, request medication or communicate on the patient’s behalf. Representative access should follow the practice’s consent, guardianship and proxy-access procedures.
Portal requests are handled more efficiently when they are complete, focused and submitted through the correct workflow.
Messaging, medication requests, forms and records may each follow a different review process. Sending a request does not confirm completion, approval or an immediate response from the practice.
Understand what information each request may require before submitting it.
Portal features and processing requirements may vary according to the patient’s care plan, account permissions and the practice’s current policies.
Nonurgent Clinical Messages
Ask a focused question about established careDescribe the main concern, when it began, relevant changes and the specific guidance being requested. New or complex symptoms may require an appointment instead of a written response.
Medication Refill Requests
Include complete medication and pharmacy informationProvide the medication name, strength, directions, preferred pharmacy and remaining supply. Refills may require monitoring, follow-up or a scheduled medication-management appointment.
Assigned Patient Forms
Complete forms before the stated deadlineReview all required fields, answer accurately and submit the form before the appointment. Incomplete intake, consent or screening forms may delay the visit or require rescheduling.
Letters and Administrative Forms
Explain the document and submission deadlineIdentify the form, requesting organization, purpose and deadline. Some documents require a clinical appointment, record review, written authorization or an administrative fee.
Viewing Released Information
Access documents made available in the portalPatients may be able to review visit summaries, instructions, selected test information, medication lists or other documents released through the portal platform.
Formal Medical-Record Requests
Use the required authorization processA complete medical record or transfer to another provider may require a separate signed authorization, identity verification and processing outside the ordinary portal-document area.
Review the request for accuracy, clarity and completeness.
A well-prepared submission can reduce unnecessary clarification and help the practice route the request correctly.
The portal view may not contain the complete designated medical record.
Documents visible online may represent selected information released for patient access. A formal records request may be needed for the full record or for transmission to another organization.
A submitted refill request does not mean the prescription has been approved or sent.
The provider may need to review appointment attendance, medication monitoring, treatment response, safety information or pharmacy details before authorizing a refill. Patients should check the portal and pharmacy for confirmation rather than assuming the request was completed.
Portal messages and uploaded documents become part of the practice’s communication and clinical workflow. Patients should use respectful, accurate language and submit only information relevant to their own authorized care.
The portal is designed for routine needs— not situations in which safety or health cannot wait.
Portal submissions may be reviewed during normal practice workflows and may require staff routing, clinical review or an appointment. Patients should never depend on the portal for immediate crisis counseling, emergency treatment or time-sensitive medical care.
When someone may be in immediate danger, do not send a portal message and wait.
Call emergency services when there is immediate physical danger, serious injury, an active suicide attempt, severe violence, loss of consciousness or another medical emergency. For suicidal thoughts, emotional distress, mental-health crisis or substance-use crisis in the United States, call or text 988.
Secure messaging does not make the portal a continuously monitored clinical service.
Availability may be affected by business hours, weekends, holidays, staff routing, provider schedules and the type of review required. Practice-specific response expectations should be confirmed before publication.
No Immediate Response Guarantee
Messages enter the normal practice workflowA portal submission may not be opened or answered immediately and should not be used when delayed review could create a safety or medical risk.
No Crisis Counseling
Written messaging cannot replace real-time crisis supportPortal staff cannot continuously monitor a patient’s safety, de-escalate an active crisis or provide the immediate response available through crisis and emergency services.
No Automatic Refill Approval
Submission begins review rather than confirming a prescriptionRefills may depend on appointment attendance, medication monitoring, treatment response, pharmacy details and the provider’s clinical judgment.
No Full Clinical Assessment
Complex symptoms may require a scheduled visitNew diagnoses, major medication changes, severe deterioration and detailed treatment decisions usually require direct clinical assessment rather than a brief written exchange.
No Appointment Confirmation by Assumption
Requests remain pending until confirmedA scheduling request does not reserve an appointment unless the patient receives a clear confirmation through the portal, telephone or another approved practice channel.
No After-Hours Monitoring Assumption
Weekends and holidays may affect reviewPatients should not assume that messages submitted after hours, during weekends or on holidays will be seen before the practice resumes normal operations.
Seek immediate or urgent help when delay could place someone at risk.
This list is not exhaustive. When uncertain whether a concern is safe to manage through routine messaging, use a more immediate clinical, crisis or emergency pathway.
Publish the practice’s actual routine response expectations before launching this page.
Do not promise a specific response time unless the practice can consistently meet it. Explain whether the timeframe uses business days, how weekends and holidays are handled and what patients should do when a routine concern becomes urgent while awaiting review.
Sending a message does not ensure that the practice has received, opened or acted on it. Patients remain responsible for using the appropriate crisis, emergency or in-person care pathway when symptoms or safety concerns cannot wait for routine portal review.
Use the patient portal to stay organized, informed and connected between appointments.
Sign in through the verified portal pathway to review available information, complete assigned forms and submit approved nonurgent requests. Use the portal only for needs that can safely wait for routine review and choose a more immediate pathway whenever safety or health cannot wait.
The patient portal is not continuously monitored and must not be used for emergencies or immediate safety concerns. Use the appropriate crisis, emergency or in-person care pathway when a concern cannot safely wait for routine practice review.
